    Cardiovascular profile of vasculitis patients at tertiary care center

    Background: The vasculitides are a heterogenous group of conditions characterized by blood vessel inflammation and necrosis. Vasculitides are relatively uncommon conditions whose etiology is still poorly understood. Objective of the research was to study the cardiovascular profile of vasculitis patients at a tertiary care centre.Methods: The present hospital based observational study was conducted in the department of internal medicine, Sher-i-Kashmir Institute of Medical Sciences (SKIMS), Srinagar, Jammu and Kashmir, India. The study had two parts: retrospective and prospective. In the retrospective part, all patients of vasculitis who were admitted or evaluated in outpatient department (OPD) from March 2012 to September 2018, were enrolled for the analysis. In the prospective part, all patients of vasculitis admitted or evaluated in OPD from October 2018 to May 2020 were enrolled for study.Results: Out of total 77 patients studied, 39 (50.6%) were prospective cases and 38 (49.4%) cases were of retrospective nature. Normal echocardiographic and electrocardiography (ECG) findings were seen in majority of all three groups. Computed tomography (CT) angio shows involvement of right subclavian artery in 9 (40.9%) patients, 8 (36.4%) patients had involvement of left subclavian artery, 6 (27.3%) patients had involvement of arch of aorta, CT angio was suggestive of involvement left common carotid artery 5 patients, 2 patients each had involvement of celiac artery, bilateral iliac, ascending aorta, normal CT angio findings. Arterial Doppler was suggestive of involvement of right common carotid artery (CCA) and left CCA in 4 (30.8%) patients each, right superior cerebellar artery (SCA) involvement in 3 (23.1%) patients, left SCA involvement in 4 (30.8%) patients, left right brachiocephalic artery (RBA), right RA and B/L UA involvement was observed in 1 (7.7%) patient each.Conclusions: The association between cardiovascular disease and a vasculitis is well documented. our study discusses the association between cv disease and vasculitis

    We report a case of peritoneal hydatidosis that occurred post laparotomy. Patient was diagnosed nine months after she had laparotomy for suspected acute appendicitis. The whole peritoneal cavity was studded with cysts. In view of diffuse involvement, patient was managed conservatively and showed response to medical therapy

    Tuberculosis of breast is a rare disease which is difficult to differentiate from carcinoma of breast. The involvement of breast can be primary or secondary to some focus in body. A case of secondary tuberculosis of right breast in a 21-year-old female from Kashmir, India, is being reported. Presentation was as a painless discharging sinus of right breast. A tubercular foci of rib was the affecting source of disease. No other evidence of tuberculosis was present in the body. Resection of involved rib segment, along with the discharging sinus, was performed. The patient had antitubercular therapy for 9 months, with no recurrence seen in followup

    Improving Scientific Literature Classification: A Parameter-Efficient Transformer-Based Approach

    Transformer-based models have been utilized in natural language processing (NLP) for a wide variety of tasks like summarization, translation, and conversational agents. These models can capture long-term dependencies within the input, so they have significantly more representational capabilities than Convolutional Neural Networks (CNN) and Recurrent Neural Networks (RNN). Nevertheless, these models require significant computational resources in terms of high memory usage, and extensive training time. In this paper, we propose a novel document categorization model, with improved parameter efficiency that encodes text using a single, lightweight, multiheaded attention encoder block. The model also uses a hybrid word and position embedding to represent input tokens. The proposed model is evaluated for the Scientific Literature Classification task (SLC) and is compared with state-of-the-art models that have previously been applied to the task. Ten datasets of varying sizes and class distributions have been employed in the experiments. The proposed model shows significant performance improvements, with a high level of efficiency in terms of parameter and computation resource requirements as compared to other transformer-based models, and outperforms previously used methods

    Multiple - Antenna System for Mobile Phones

    The Multiple Antenna System proposed in the following paper is based upon two types of patch antennas working in different frequency ranges. The antennas are classified into Main and Subsidiary antenna. The Main antenna shall work for the GSM ranges of 2G, 3G and 4G covering the bandwidth from 0.7 to 2.5 GHz. A subsidiary antenna is dedicated to work in the range of 2200-2600 MHz hence providing assistance in the LTE 2350 MHz and 2550 MHz bands and also in Bluetooth range. The whole system is designed on a PCB of dimensions 135x66.1x1.5 mm3. Effectively increasing the isolation between the antennas the system may be optimal for modern day wireless communications.     Urological implications associated with the use of recreational drugs: A narrative review

    About 275 million people worldwide aged between 15 and 64 years used drugs at least once since 2016. Initial estimations suggest that 13.8 million young people between 15 and 16 years used cannabis every year. Recreational drug use contributes significantly to mortality as well as physical and mental health problems. A number of urological complications can arise from the use of common and emerging recreational drugs which can present as wide spectrum affecting lower and upper urinary tracts, kidneys, sexual organs as well as sexual dysfunction. In order to effectively manage these issues, urologists need to be cognizant of these complications in their patients, particularly among youths. This review attempted to consolidate available data and provide insight into this issue; however, further population-based epidemiological studies are needed to provide necessary guidelines

    Changing histological spectrum of adult nephrotic syndrome in comparison to previous study: single centre analysis

    Background: Glomerular diseases are an important cause of chronic renal failure in developing countries. The spectrum of diseases causing nephrotic syndrome is changing globally in the last few decades. Methods: Patients in the age group 18-60 years with nephrotic syndrome were consecutively included in the study. Renal biopsies were performed in all patients and were subjected to light microscopy, immunofluorescence (IF) and electron microscopy (EM). Results: 189 patients (67% males) were included in the study. The mean age was 43 years. Primary glomerular diseases accounted for 92.5% of cases while lupus nephritis was the most common secondary glomerular disease. Focal segmental glomerulosclerosis (FSGS) accounted for 28.6% of primary glomerular diseases making it the most common cause of nephrotic syndrome. It was followed by membranous glomerulonephritis (MGN) in 13.2%, membranoproliferative glomerulonephritis in 11.2%, diffuse proliferative glomerulonephritis in 10.6% and minimal change disease in 9.5%. Conclusions: The biopsy diagnosis of FSGS has increased considerably in last few decades and it is now the most common cause of nephrotic syndrome in adults in North India. MGN is the most common lesion in patients over 40 years of age
